HPSM Selects Upward Health To Provide Home-Based Care

HPSM is a local, non-profit County Organized Health System in California founded in 1987 with over 140,000 members, representing about 20% of San Mateo County’s population. Nearly 9,000 HPSM members are dually eligible for Medicare and Medi-Cal. HPSM sponsors several unique programs to address social determinants of health, from home-based health care to programs that help address social isolation and assistance finding housing.

HPSM first launched its HomeAdvantage Program in 2016. It has three service tiers: a longitudinal program, an on-demand service, and a short-term program called FocusCare that provides follow-up care after a hospitalization or emergency department visit. The program had been provided by Landmark Health since 2016. HPSM contracted with Upward Health for all three service tiers:

- The HomeAdvantage longitudinal program serves high-acuity members with regular in-home care through a multidisciplinary team and care management services to connect members to care and community resources.
- The HomeAdvantage OnDemand service consists of annual in-home visits, access to in-person urgent care, and telephone access to healthcare.
- HomeAdvantage FocusCare enrolls members in the longitudinal program for 90 days after an event such as an inpatient admission, emergency department, or hospital visit.

Upward Health partners with health plans and other risk-bearing entities to provide multi-disciplinary in-home care services. It has contracts in nine states to serve people in California, Georgia, Louisiana, Massachusetts, New Jersey, New York, Pennsylvania, Rhode Island, and Texas. For each consumer served, Upward Health provides an initial comprehensive evaluation and an ongoing care plan for each health care consumer. Upward Health uses technology to deliver facilitated video visits and remote monitoring of chronic conditions that facilitate Upward Health’s clinical professionals in providing medication orders and diagnostic testing.
