Member Education Materials

HPSM produces a range of print materials to educate our members about specific health topics and programs we offer. These materials are compiled here and presented as downloadable PDFs that you can read online or print out. Materials available to be downloaded in other languages can be found here. Our “Be Healthy” member newsletters are filled with tips and resources about different health topics. Each eight-page booklet is a practical guide that presents information in an easy-to-use printed format. Anyone can read and download the newsletters here. Programs for older adults and members with disabilities

HPSM partners with several organizations in and around San Mateo County to provide programs focused on supporting the health and well-being of our members who are older or have disabilities. Learn more about these programs and who is eligible below.

HomeAdvantage 

This program provides eligible CareAdvantage members with routine home visits, urgent on-call treatment, 24/7 phone access to providers, transition support after a hospital visit and more. Learn more.
