We offer special programs to support all our members: from people living with specific chronic conditions to those who just want to be their healthiest. We offer many of these programs in collaboration with our local community partners. Our special programs include:
Cancer Screening Program
Earn Cinemark Prepaid Movie Tickets when you get screenings for breast or colorectal cancer.
Diabetes Prevention Program
If you have prediabetes, lower your risk for Type 2 diabetes with weekly group sessions.
Care Management
Get help meeting your health goals with personalized support from a Care Manager.
HomeAdvantage
A home-visit service with urgent on-call treatment, 24/7 phone access and transition support after a hospital visit.
Matrix
Home-based health assessments performed by licensed nurse practitioners in the comfort of members’ homes.
Wider Circle
In-person social events for older adults who want to make new friends and have more fun.

If you live in San Mateo County, have Medicare Parts A & B and full-scope Medi-Cal through HPSM
CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
