New address or contact information?

If you move and/or change your contact information (phone number, email), you must tell both HPSM and either the San Mateo County Human Services Agency at 1-800-223-8383 or Social Security Administration at 1-800-772-1213. This will ensure that you continue to get important information about your Medi-Cal coverage status.

To report your new information with HPSM, you can log in to the Member Portal or call the number below if you need assistance.

Want to speed up your journey to optimal health? Then join HPSM’s no-cost Care Management Program! A Care Manager will be your personal health coach. They’ll work directly with you and your healthcare providers to design a care plan based on your personal health priorities. This helps you:

  • Identify and solve problems that keep you from meeting your health goals
  • Develop healthy self-care habits, from exercising and eating well to managing stress
  • Connect with helpful resources and programs offered by HPSM and our community partners
  • Manage any complex medical conditions or critical events you or your family may face
