Answer a few questions to get your personal care plan 

To make sure you get care that meets your needs, we need to know about your health. The HRA survey is a set of questions about your health. It gives us the answers we need to help you. After you take the HRA, you’ll get a personal care plan designed to meet your unique healthcare needs! It will cover medicines, visits with providers, nutrition, exercise and more.

Please take the HRA even if you see your doctor regularly. It only takes 20 minutes to fill out. Your answers will not change your coverage. Both your HRA and care plan are confidential.

Two easy ways to answer the survey:

By phone

  • Call 1-888-783-3035 or 650-616-5035 (TTY: 1-800-735-2929 or dial 7-1-1) Monday – Friday from 8:00 a.m. – 8:00 p.m.
  • A representative from our Member Assessment Unit will ask you the survey questions and record your answers.

By mail

  • Open the envelope with the HRA survey inside (It is a 9in x 12in envelope with a picture of a car dashboard on it)
  • Fill out the form by filling in the bubbles with a black or blue pen
    • Do not use a pencil, permanent marker or highlighter
    • Write your responses in print
    • The form is double-sided: fill out both sides
  • Mail the survey to HPSM in the postage paid that came with it

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Benefits and co-pays may change on January 1 of each year.
