New address or contact information?

If you move and/or change your contact information (phone number, email), you must tell both HPSM and either the San Mateo County Human Services Agency at 1-800-223-8383 or Social Security Administration at 1-800-772-1213. This will ensure that you continue to get important information about your Medi-Cal coverage status.

Matrix Medical Network

Health assessments in the comfort of your home

HPSM partners with Matrix Medical Network to provide comprehensive, convenient home-based assessments for eligible CareAdvantage members. These 45-to-60-minute voluntary visits can identify health risks early when they may be easier to treat. Assessments are performed by board-licensed Matrix nurse practitioners (NPs) in the comfort of your home. They focus on three core pillars of health:

  • Physical NPs perform health exams and can collect samples for lab-work. They also review your personal and family health history.
  • Emotional NPs listen to your unique story to understand you as a person. They can also answer your health questions.
  • Social NPs connect you with resources you may need, such as food banks, transportation or housing support.

Within a month after your visit, you will get an easy-to-read Personal Health Summary. This will explain what you can do to get and stay healthier. To coordinate your care, Matrix also shares this report with your primary care physician and HPSM.

Eligibility

If you are eligible for Matrix home-based health assessments, you will get a letter from HPSM. It will encourage you to schedule a visit. Your provider may also recommend you for this service based on an assessment.

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
