What You Can Do Using HPSM’s Member Portal 

HPSM’s Member Portal lets you get things done anytime from anywhere without having to call HPSM. With just a few clicks on your computer, tablet or smartphone, you can:

  • Select or change your primary care provider (PCP)Member Portal landing page with border_400X190
  • Check your benefits and eligibility
  • Change your address or phone number
  • Print or order a new HPSM Member ID Card
  • Check your immunization records
  • Complete your Initial Health Assessment (IHA)

Start using the Member Portal 

  • If you’ve already created a profile, just log in by entering your username and password.
  • If you haven’t created a profile, create one now. It’s easy! Just type in a few pieces of information, then log in by entering your new username and password.
