Cancer Screening Program for CareAdvantage Members

Cancer screening tests save lives. Getting screened can help find abnormal cells or cancer at an early stage before symptoms appear. When cancer is found early, it may be easier to treat or cure. Because we want you to stay healthy, HPSM is offering CareAdvantage members Target GiftCardsTM for getting the cancer screenings they need.

 Complete a breast cancer screening and get a $25 Target GiftCard

A mammogram can find lumps in the breast before they can be felt with the fingers. CareAdvantage members who are due for a mammogram and complete one will get a $25 Target GiftCard. If you are:

  • 40 to 50 years old: Ask your primary care provider about your specific risks for breast cancer and when to get your first mammogram.
  • 50 to 75 years old: Get a mammogram every one to two years. Contact your PCP to see if you are due for a mammogram. If you are due, ask how to schedule the appointment.

 Complete a colorectal cancer screening and get a $25 Target Gift Card

Different screening tests are used to find signs of colorectal cancer. CareAdvantage members who are due for colorectal cancer screening and complete one will get a $25 Target GiftCard. If you are:

  • 45 to 50 years old: Get your first colorectal cancer screening. Ask your PCP how often you should get screened and which screening test is right for you.
  • 50 to 75 years old: Continue to get screened. Ask your PCP how often you should get screened and which screening test is right for you.

No action is needed to receive the gift card. Your Target GiftCard will be shipped to your home address and should arrive within 90 days of your screening. If you do not get it by then, call HPSM’s Health Promotion Unit at 650-616-2165.

CareAdvantage Dual Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Enrollment in CareAdvantage depends on contract renewal.
