Primary Care ACT
Assessments, coaching and technical assistance
Primary Care ACT is part of our larger Primary Care Investment Strategy (along with our provider grants program). This initiative helps our network primary care practices build and strengthen their team capabilities across the population health building blocks of empanelment, access, data and models of care.
After completing assessments, participating practices get hands-on one-on-one coaching and group-based support. This helps practices boost population health skills, raise quality scores, strengthen team culture and close care gaps on key HEDIS measures. Practices that complete practice assessments also get incentive payments of up to $10,000.

Participating in Primary Care ACT
To complete the assessment, you will need to fill out:
- The Population Health Management Capabilities Assessment Tool (PhmCAT) by November 14, 2025. If you cannot meet that deadline, email Scott Fogle, Manager of Strategic Network Investments to request an extension.
Once the assessment forms are complete:
- A PHLC Coach will reach out to schedule an in-person “calibration” visit with your team that will take place by January 2026
- After the meeting, your practice will get a one-time incentive payment ranging from $2,500 to $10,000 (see the Quick Start Guide for details)
If you qualify based on your PhmCAT assessment, your team will be invited to join a practice cohort for evidence-based coaching. Primary Care ACT Quality Practice Coaches:
- Are local and provide five to ten hours of 1:1 tailored virtual and in-person support per month based on need
- Facilitate virtual group support meetings with other primary care practices that are participating in Primary Care ACT
- Focus on the challenges, opportunities and goals that matter most to your practice
- Have at least 10 years of professional coaching experience
- Include both quality improvement specialists and specific subject matter experts
Technical assistance is provided during both:
- Assessment – Webinars and office hours support your team in taking the assessment.
- Coaching – Our virtual training platform offers on-demand core content eLearning modules. Virtual peer group meeting facilitators can also provide technical assistance.
