Primary Care ACT

Primary Care ACT (Assessments, Coaching and Technical Assistance) is part of our larger Primary Care Investment Strategy to help our network tackle the primary care crisis and lay the groundwork for advanced primary care. Primary Care ACT helps our network of primary care practices build and strengthen their core capabilities that improve access, fortify primary care teams, and lead to better and more equitable health outcomes for HPSM members.

About Primary Care ACT

This three-year practice transformation initiative is facilitated by our esteemed program partners, PopHealth Learning Center and University of California, San Francisco (UCSF) Center for Excellence in Primary Care. It offers: 

The Assessment phase was completed in early 2026. In this phase, more than half of HPSM’s primary care provider network — 45 practice sites within 23 organizations — completed initial assessments using the tools below:

Following completion of the assessments, 15 organizations have been selected as a cohort for evidence-based coaching. Primary Care ACT practice coaches:

  • Have at least 10 years of professional coaching experience
  • Are local to the Bay Area
  • Facilitate a tier-based curriculum for participating practices to become more population focused
  • Provide five to ten hours per month of 1:1 virtual and in-person support to select practices tailored to the individual practice’s priorities, while reinforcing the core curriculum
  • Focus on the challenges, opportunities and goals that matter most to practices

From June 2026 through June 2028, all participating practices are eligible for technical assistance through the following synchronous and asynchronous modalities:

  • Practice Tracks – longitudinal, small group learning organized by practice type and assessment results to build population health management capabilities
  • PopHealth+ Online Modules- Bite-size asynchronous learning modules, providing access to population health management curriculum
  • Connections and Support- Topics driven by challenges identified by practices or opportunities for peer learning
  • Training and Office Hours- Informal, flexible support or 1:1 guidance
  • Subject Matter Experts- Share evidence-based and promising practices for managing care within specialty areas
