About HPSM

Incentive Programs

CalAIM incentive programs support our goals of increasing delivery system infrastructure, improving access to services, bridging silos across systems, reducing health disparities and promoting health equity. To reach these goals, HPSM partners closely with our local CalAIM program providers and County agencies. 

The incentives include:

  • The Incentive Program Plan (IPP), which allocates funds to providers to ensure we can deliver, expand and improve our ECM and CS offerings – Learn more
