Wider Circle 

HPSM partners with Wider Circle, a neighborhood-based health organization that helps older adults make new friends and learn healthier habits by attending fun social events in their community. This extra benefit available to eligible CareAdvantage members helps address the problem of social isolation that disproportionately affects older adults.

How Wider Circle works

1. Try out a local meeting

Check out an introductory meeting to learn more, have a snack and meet fellow members.

2. Join a neighborhood group

If you enjoyed your first meeting or two, sign up for the free Connect for Life® program. Each week, you can choose the events and topics that interest you most.

3. Grow your circle with lifelong membership

Enjoy even more activities, wisdom-sharing and friendship with chapter membership in your area

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
