CareAdvantage Dual Eligible Special Needs Plan (D-SNP) 2026

How Can I Keep My Healthcare Benefits?

To keep your healthcare benefits, it’s important to stay informed and act quickly when you’re contacted to prevent delays or loss of coverage.

What should I do when I get mail from HSA or DHCS?

When you get mail you get from San Mateo Human Services Agency (HSA) or the California Department of Health Care Services (DHCS), open it, read it carefully and respond as soon as you can by either:

  • Filling out the Medi-Cal renewal form: include any documents (pay stubs, etc.) and return them by the due date. If you miss the due date, make sure you still send the information. The information may still be accepted within 90 days.
  • Complete your Medi-Cal renewal online: log in to the Benefits Cal portalIf you do not have an account, create one.

When HSA gets your packet or online renewal, they will mail you a letter saying your Medi-Cal was renewed or asking for more information.

If you have any questions about your Medi-Cal renewal:

If you lose your coverage and believe you are still eligible, ask HSA to review your case.

How can I take my healthcare benefits with me when I move?

HPSM is a public health plan for people who live in San Mateo County. If you are moving out of San Mateo County, you need to let healthcare agencies know your new address in order to keep your coverage. Here is what you need to do before you move.

  • For Medi-Cal get an inter-county transfer (ICT) from the Human Services Agency (HSA). Call 1-800-223-8383 or log in to the BenefitsCal Portal
  • For Medicare give your new address to the Social Security Administration (SSA). With Medicare Parts A & B, you can:
    • Call 1-800-772-1213 (TTY: 1-800-325-0778) Monday through Friday, 8:00 a.m. to 7:00 p.m.
    • Update your address online.
    • Visit your local SSA office: find a location near you.

Make sure HPSM has your most current address
