Here’s a brief overview of the four grants:

Primary Care Team Expansion Grant

Hire and integrate new interprofessional team members. The funding provides the upfront dollars to hire, operationalize and sustain new interprofessional team member roles that will increase primary care team capacity.

Who should apply? Primary care organizations looking to expand their primary care teams’ capacity with new roles.

Recruit and retain primary care providers and medical assistants. This funding helps practices stabilize their primary care teams by offering competitive sign-on bonuses to augment compensation packages for primary care providers and medical assistants.

Who should apply? Primary care organizations hiring primary care providers and medical assistants who want additional funding for recruitment and retention.

Retain providers by enhancing resilience through sabbatical. This funding rewards long-term primary care providers for their service and commitment to HPSM members with paid time off.

Who should apply? Primary care organizations looking to expand their retention strategy by rewarding tenured providers with sabbatical.

Develop pilots and programs that improve primary care teams’ capacity, bandwidth and joy.

Who should apply? Primary care organizations with an innovative idea that could improve quality of care for HPSM members and quality of life for primary care teams.

San Mateo County has one of the greatest income disparities in the country. The cost of living can be extremely high, especially for families raising children. There is a growing body of evidence that family income impacts children’s ability to pay attention, self-regulate, perform well in school and achieve long-term goals. To support our members, HPSM, in partnership with several organizations, has launched the new San Mateo County Baby Bonus pilot program. 

The Baby Bonus pilot program is designed to evaluate the impacts of regular guaranteed income and early childhood supports on the health and well-being of parents and their new baby. It includes a randomized control trial that will compare social and health outcomes among different groups. The data collected by the program and stories collected from families will be used to inform policy changes aimed at reducing childhood poverty. 

What are benefits for members? Program participants will be given one or more of the following supports: 

• Cash aid 
• Care coordination through a Community Health Worker (CHW) 
• Incentives to participate in surveys

Who is eligible? The Baby Bonus program is for a limited number of San Mateo County parents who receive Medi-Cal through HPSM. There is no open application process for enrollment. Eligible HPSM members will be contacted after giving birth to their baby, either in the hospital or by HPSM.

HPSM is conducting outreach to support better health outcomes for members managing diabetes. Outreach includes recommendations for proactive care, which may prompt questions during primary care visits. Please be prepared to assist HPSM members if they have questions about:

• Statins: A prescription medication that can help lower the risk of heart attack and stroke by lowering cholesterol for at-risk members. Statins are a covered benefit available to HPSM Medi-Cal and CareAdvantage members of any age with diabetes who have high cholesterol or are 40-75 years old with diabetes and normal cholesterol.  
• 90-day medication supply:  HPSM Medi-Cal and CareAdvantage members can save time by filling a three-month supply of insulin and other prescription medications, including those prescribed for managing diabetes. CareAdvantage members with copays get an extra benefit—90-day supplies cost the same as 30-day supplies, so they can save money by choosing the 90-day option.
• Mail-Order Pharmacy Benefit: CareAdvantage members can get no-cost home delivery for insulin and other prescription medications.
