Planning To Move? Take Your Healthcare Benefits with You.

HPSM is a public health plan for people who live in San Mateo County. If you are moving out of San Mateo County, you need to let healthcare agencies know your new address in order to keep your coverage. Here is what you need to do before you move.

For Medi-Cal — get an inter-county transfer (ICT) from the Human Services Agency (HSA). Call 1-800-223-8383 or log in to www.benefitscal.com.

For Medicare — give your new address to the Social Security Administration (SSA). With Medicare Parts A & B, you can:

CareAdvantage members moving outside San Mateo County will lose their CareAdvantage coverage. Medicare Advantage Dual Eligible Special Needs Plans like CareAdvantage are currently available in 11 other California counties (Fresno, Kings, Los Angeles, Madera, Orange, Riverside, Sacramento, San Bernardino, San Diego, Santa Clara and Tulare). Plus, D-SNPs will be offered in more counties starting in 2026. To find D-SNPs in the county you’re moving to, search the State Health Insurance Assistance Program (SHIP) website. To enroll in another D-SNP, contact the plan.
