Get Support for Chronic Health Conditions

Chronic health conditions, such as diabetes and hypertension (high blood pressure), are long-lasting diseases that need ongoing care. As you approach the end of the year, learn more about several programs and services HPSM covers at no cost to our members to help you manage your chronic health conditions.

General tips and health information

  • Visit your primary care provider (PCP) at least twice a year. Your PCP can help you manage your chronic condition(s), set goals and answer any questions.
  • Visit HPSM’s Health Tips webpages for general health information, tips and resources for managing chronic conditions, including diabetes.
  • Take your medicines as discussed with your healthcare provider. Staying organized can help make sure you take your medicines correctly. Learn more.

Medicines and supplies for managing chronic health conditions

  • Ask your PCP or pharmacy for a 90-day supply of your prescription medicines, including those prescribed for managing diabetes and hypertension. HPSM members can save time by making fewer trips to the pharmacy.
  • Talk with your PCP if you would benefit from having supplies at home to help manage your chronic health condition, such as a blood pressure monitor or glucometer (a tool used to check your blood sugar). Your PCP can talk to you about the options available and help you order supplies.
