Medicines and supplies for managing chronic health conditions
- Ask your PCP or pharmacy for a 90-day supply of your prescription medicines, including those prescribed for managing diabetes and hypertension. HPSM members can save time by making fewer trips to the pharmacy.
- Talk with your PCP if you would benefit from having supplies at home to help manage your chronic health condition, such as a blood pressure monitor or glucometer (a tool used to check your blood sugar). Your PCP can talk to you about the options available and help you order supplies.
