The approach of a new year is a golden opportunity to encourage your patients to adopt healthy habits. That’s why we encourage you to refer HPSM members to the support programs and services we offer to help them stay healthy.
Diabetes Prevention Program (DPP)
HPSM provides DPP to eligible Medi-Cal and CareAdvantage members with pre-diabetes at no cost to them. This evidence-based 12-month lifestyle program is designed to prevent or delay the onset of type 2 diabetes and features:
- One-hour in-person or online group sessions led by a trained Lifestyle Coach who provides support for healthy eating, regular exercise and weight management
- Educational materials to help people meet their personal health goals
- Support from other participants
Sessions are weekly for the first six months and monthly in the last six months. DPP participants can use HPSM’s Ride Benefit to get to their in-person DPP sessions and back home.
Eligible Medi-Cal members who participate in DPP can also enroll in HPSM’s Fitness Membership Program at no cost while they’re in DPP.

CareAdvantage Fitness Membership Program
Regular exercise can help your patients feel better and stay healthy! HPSM offers a YMCA membership to CareAdvantage members at no cost. The membership includes access to:
- YMCA locations throughout San Mateo / Santa Clara Counties and San Francisco that may include exercise equipment, basketball courts, indoor or outdoor fitness studios, and indoor or outdoor swimming pools
- More than 600 in-person and online group exercise classes led by certified instructors in yoga, Pilates, Zumba, cycling, aqua-fitness and more
- In-person and online wellness coaching to help members meet their heath goals
Encourage your CareAdvantage patients to sign up for and use their YMCA membership! They can either:
- Visit a YMCA location near them and show their HPSM CareAdvantage member ID card
- Sign up online by selecting a YMCA location near them and clicking the “Enroll Today” button
CareAdvantage members enrolled in the Fitness Membership Program can receive 12 one-way rides per month to YMCA gyms.

Services and resources for eating healthy
HPSM registered dietitians
HPSM’s in-network registered dietitians can offer our members advice on nutrition and healthy habits. Some registered dietitians may require a provider referral. Please be prepared to assist HPSM members who request these referrals.
Medically supportive meals
Medi-Cal and CareAdvantage members with certain chronic conditions are eligible to have meals specially prepared to meet their nutritional needs delivered to their homes. Please be prepared to assist HPSM members who request referrals to this service. Either:
- Visit our Community Supports Provider webpage and complete a provider referral form, then email it to Mom’s Meals
- Direct members to call HPSM’s Integrated Care Management Unit at 615-616-2060
