At HPSM, we encourage our members to visit their primary care provider (PCP) for a well visit every year – even if they feel healthy. Well visits offer a private space for our members to talk about their health, ask questions and stay up to date with recommended vaccines and screenings. HPSM is committed to helping our members stay on track with their well visits. For instance:
- HPSM’s Well Baby Package is offered to our Medi-Cal members who recently had a baby when they sign up for our Baby+Me program. The package includes health tips and tools to support their baby’s first six years, such as information on when to schedule their baby’s well visit and vaccines.
- We conduct telephonic outreach to parents/guardians of members ages 0-24 months to provide reminders to schedule their child’s well visits.
- Every month, we send reminder letters to Medi-Cal members turning 3-21. These letters include information on the benefits of a well visit, what the visit entails and how to schedule an appointment.
- Our annual member newsletters remind members to schedule their well visit.
- HPSM’s member webpages provide information on the importance of well visits across different age groups:
- Child Health
- Teen Health
- Adult Preventive Care
- HPSM’s Teen Wellness Rewards Program offers an incentive to eligible members for completing their well visit. More information is provided below.
Teen Wellness Rewards Program
Medi-Cal members ages 12-21 who are assigned to San Mateo Medical Center Clinics can get a $25 gift card for completing their well visit. Eligible members who complete a well visit will receive a letter in the mail with instructions on how to select their $25 gift card. We encourage you to:
- Remind your patients to schedule their annual well visit.
- Be prepared to assist HPSM members if they have questions about well visits.
- Inform Medi-Cal members ages 12-21 assigned to San Mateo Medical Center Clinics they can get a $25 gift card for completing their well visit.
