Teens & Young Adults: Check In On Your Health

Even if you feel healthy, getting a yearly checkup (well visit) with your primary care provider (PCP) is important. HPSM members get these well visits at no cost. At your well visit, your PCP can:

  • Answer questions about your physical, mental and sexual health.
  • Offer you screenings and vaccines.
  • Make a plan to address any health concerns.
  • Share tips and advice to help you stay healthy.

Call your PCP and schedule a well visit. Don't have a PCP or don't know who your PCP is? Choose one or find out who they are.

Make sure you're ready for your visit

  • Bring your HPSM ID card and questions about your health.
  • Need a ride? Use your HPSM Ride benefit.
  • Learn more about well visits.
  • If you are under 18, you will need a parent or guardian at your well visit. However, you can ask your PCP for time alone to talk about sensitive topics.

Teen Wellness RewardsHPSM Medi-Cal members ages 12-21 assigned to San Mateo Medical Center clinics can get a $25 gift card after completing a well visit. Call your PCP and schedule your well visit!

Are you 17-18 years old? It may be time to choose a new PCP for your adult care. This can be an important step in taking charge of your health. Ask your current PCP if you need to choose a new one.
