HPSM’s Population Health Management (PHM) Program offers five support programs at no cost to help members reach their health goals. Each program includes direct support, information and resources.

- Asthma Management Program gives members with asthma information about the causes of asthma and how to manage symptoms. It also provides members with supplies to reduce asthma triggers at home.
- Complex Case Management Program is for all members with one or more ongoing health conditions. Participants work with a Care Manager to create a care plan based on their health goals. This helps with practicing self-care, finding useful resources and managing complex medical conditions or addressing medical problems that need urgent attention.
- Care Transitions Program is for members who have been sent home from a facility (hospital or skilled nursing). Members get help following a care plan that supports their ability to live and thrive at home and avoid going back to the hospital.

- Baby + Me Program supports HPSM Medi-Cal members who are pregnant or recently had a baby. Participants can earn up to $100 in gift cards for going to two visits!
- Diabetes Prevention Program (DPP) can help eligible HSPM Medi-Cal and CareAdvantage members with prediabetes lower their risk of getting type 2 diabetes. This 12-month program includes one-hour online or in-person group sessions led by a trained Lifestyle Coach. They provide support for healthy eating, regular exercise and more. Participants also receive educational materials to help meet their health goals.
