Managing Chronic Health Conditions

Chronic health conditions, like diabetes and hypertension (high blood pressure), are long-lasting diseases that need ongoing care. HPSM offers several programs and covered services at no cost to help you manage these conditions.

To help manage chronic health conditions:

  • Visit your healthcare provider at least twice a year. They can help you set goals and answer your questions. Ask whether having supplies at home will help manage your chronic health condition. This could include a blood pressure monitor or glucometer (a tool used to check your blood sugar).
  • Take your medicines as discussed with your healthcare provider. You can also ask your healthcare provider or pharmacy for a 90-day supply of your prescription medicines.
  • Learn more about managing chronic health conditions. Visit our Health Tips to learn about topics related to chronic health conditions, like diabetes, healthy eating, exercise, managing medicines and more.
  • Call HPSM and get connected to:
    • Wellness programs, services and community resources — call the Health Promotion Unit: 650-616-2165.
    • A care manager to coordinate your providers, medicines and more — call the Integrated Care Management Unit: 650-616-2060. Learn more about the Care Management Program.

CareAdvantage members can get other no-cost services to help manage chronic conditions, like:

  • Over-the-counter (OTC) coverage for medicines and more
