It’s recently come to our attention that primary care providers (PCPs) have questions regarding Interdisciplinary Care Team (ICT) meeting invites and member care plans that they have received from HPSM via fax. These questions are answered below.
ICT invites
You may receive ICT invites for assigned members, including those who may not have had an appointment at your clinic. If you want to attend the ICT meeting, call 650-616-2060 within 72 hours of receiving the invite. If you have any clinical concerns, you can also fax them to 650-829-2047.

Member care plans
You may receive care plans about any assigned member, including those who may not have had an appointment at your clinic recently. Our clinical and care management teams share member care plans with you to keep you informed of important health information regarding your assigned HPSM members. Plus, as a managed health plan, we are required to fax care plans to the PCP on file.

For more information about your assigned members, including care gaps, familiarize yourself with the many eReports in our Provider Portal. Sign up for eReports and learn about the financial incentives you can earn for closing care gaps.
If you get care plans at the wrong fax number or aren’t receiving them, update your contact information. Changes submitted will be reflected in HPSM’s records within 30 days and often much sooner.
