Supporting Members with Diabetes

HPSM is conducting outreach to support better health outcomes for members managing diabetes. Outreach includes recommendations for proactive care, which may prompt questions during primary care visits. Please be prepared to assist HPSM members if they have questions about:

  • Statins: A prescription medication that can help lower the risk of heart attack and stroke by lowering cholesterol for at-risk members. Statins are a covered benefit available to HPSM Medi-Cal and CareAdvantage members of any age with diabetes who have high cholesterol or are 40-75 years old with diabetes and normal cholesterol.  
  • 90-day medication supply:  HPSM Medi-Cal and CareAdvantage members can save time by filling a three-month supply of insulin and other prescription medications, including those prescribed for managing diabetes. CareAdvantage members with copays get an extra benefit—90-day supplies cost the same as 30-day supplies, so they can save money by choosing the 90-day option.
  • Mail-Order Pharmacy Benefit: CareAdvantage members can get no-cost home delivery for insulin and other prescription medications.
