Primary Care Investments: Grants and Practice Assessments

This past April, HPSM launched four new grants to support our primary care network. We’re proud to announce we’ve already received nearly two dozen applications. We’ve also already approved over $4 million in disbursements to primary care practices like yours. 

Our four Primary Care Provider Grants are:

  1. Primary Care Team Expansion Grant – Hire and integrate new interprofessional team members
  2. Core Team Stabilization Grant – Recruit and retain primary care providers and medical assistants
  3. Provider Sabbatical Grant – Retain providers by enhancing resilience through sabbatical
  4. Custom Pilot Grant – Design pilots and programs that improve primary care team capacity, bandwidth and joy

Building on the momentum of the Primary Care Provider Grants, the next phase of HPSM’s Primary Care Investment Strategy – Primary Care ACT – is about to begin. These will be critical to improving our understanding of our primary care network and deploying our investments to better meet your needs. Watch this short video to learn more about this initiative.

HPSM will provide financial incentives to practices that complete the assessments. Our goal is to have our entire primary care network participate in these assessments, so please look for emails from HPSM and our partners at the PopHealth Learning Center who will be conducting outreach in October.
