You may receive a request from the Health Plan of San Mateo (HPSM) to complete the Health Risk Assessment (HRA) form for CareAdvantage members in your facility. As you receive our HRA, please assist the Care Advantage member in completing the form.
HRAs are a critical component of the CareAdvantage program as they help identify members’ health care needs, risks, opportunities for care coordination, and more. Please note: a members’ responses to the HRA will not change or affect their CareAdvantage coverage, benefits, or eligibility. The assessment is used only to help identify health care needs and support care coordination.
Members residing in ICFs/DDs, care homes, and SNFs often rely on facility staff/administrators to assist them with completing assessments. Provider awareness and cooperation will help ensure:
Accurate and timely completion of HRAs
Better identification of members’ medical and social needs
Improved care coordination and care planning
Compliance with Centers for Medicare and Medicaid Services (CMS) requirements
