Support To Help You Stay Healthy

HPSM offers special support programs and resources to help you stay healthy.

HPSM’s Asthma Program

Breathe California now provides support services to HPSM members who have asthma. Their Home Visit Program offers two sessions that include:

  • Information about what causes asthma.
  • How to manage symptoms with medicine and an Asthma Action Plan.
  • A home assessment to make improvements such as removing mold and increasing ventilation.
  • Supplies such as air filters, vacuum cleaners, dehumidifiers and hypoallergenic mattress and pillow covers.

To participate in the Home Visit Program, call HPSM’s Health Promotion Unit at 650-616-2165.

Cancer Screenings Rewards Program

CareAdvantage members can now get a Cinemark Prepaid Movie Ticket for getting their breast or colorectal cancer screenings. That means you can get two Cinemark Prepaid Movie Tickets if you get both screenings! Once you get your screening, the Cinemark Prepaid Movie Ticket will be shipped to the home address that you provided to HPSM. It should arrive within 90 days of when you got your screening. Talk with your primary care provider (PCP) if you are not sure when you should get your next breast or colorectal cancer screening.

HPSM’s Educational Resources

Each of our eight-page “Be Healthy” newsletters serves as an easy-to-use guide to a specific health topic.

  • The Diabetes Newsletter has tips to help manage diabetes, a diabetes control checklist and online/local resources.
  • The Taking Action for Your Mental Health Newsletter helps people recharge, get care and develop a more positive mindset.
  • The Quit Tobacco Newsletter includes the benefits of quitting, the six steps to building a quit plan and other resources.

These newsletters are available in multiple languages. To view or download them, go to www.hpsm.org/member-education and choose the topic you want to learn about. To have print copies mailed to you, call the Health Promotion Unit at 650-616-2165.
