HPSM is a public healthcare plan for people who live in San Mateo County. If you move out of the county, you will no longer be an HSPM member. But you can keep your Medi-Cal and Medicare coverage by letting healthcare agencies know your new address before you move.
CareAdvantage members moving outside San Mateo County will lose their HPSM CareAdvantage coverage. Medicare Advantage Dual Eligible Special Needs Plans (D-SNP) are available in other California counties and states – you may be able to join another one.
