Do you provide gender-affirming care in your practice?
If so, please complete this survey so we can identify providers who offer gender affirming services and inclusive spaces for LGBTQIA+ members. This will help us make gender affirming care more accessible to HPSM’s members and help you get more LGBTQIA+ patients who need the support you offer.
Before filling out the survey, please review these definitions:
- Sexual orientation: An individual’s enduring physical attraction to members of the same and/or other gender(s).
- Gender identity and expression: An internalized realization of one's gender that may not be manifested in their outward appearance (gender expression) or their place in society (gender role).
- Transgender: People who have gender identities and/or gender expressions that differ from what is typically associated with the gender they were assigned at birth.
The survey will ask if you provide gender-affirming services, which refers to the following procedures:

What is gender affirming care? Gender-affirming care is defined as a range of social, psychological, behavioral, and medical interventions to address incongruence between a person’s gender assigned at birth and their gender identity. See the full definition from Medi-Cal.
