Help Deliver Gender-Affirming Care to HPSM Members

Do you provide gender-affirming care in your practice?

If so, please complete this survey so we can identify providers who offer gender affirming services and inclusive spaces for LGBTQIA+ members. This will help us make gender affirming care more accessible to HPSM’s members and help you get more LGBTQIA+ patients who need the support you offer.

  • Transgender: People who have gender identities and/or gender expressions that differ from what is typically associated with the gender they were assigned at birth.

The survey will ask if you provide gender-affirming services, which refers to the following procedures:

  • Behavioral Health
  • Facial surgery
  • Feminizing genioplasty
