Get One-on-One Support to Meet Your Health Goals

HPSM’s Care Management Program is a vital tool to help improve your healthcare by giving you a Care Manager for all your healthcare needs. Your Care Manager is a nurse or social worker who gets to know you and what you need. Together, you create a Care Plan that helps you stay on track to your best health.

We help you create your own personalized care plan that includes your healthcare goals and how you can meet them. You can update the plan as your needs change.

Your Care Manager helps you:

  • Use services covered by your benefits.
  • Find providers and make appointments.
  • Arrange no-cost rides to healthcare visits.
  • Get special care after you leave the hospital.
  • Connect with helpful services like food and housing support.

Your Care Team can include your Care Manager, caregivers, providers and other health professionals who you choose.

  • Your Care Manager works with your Care Team so everyone knows how to meet your needs. Team members share key information with each other, like your test results and medicines. This is especially helpful if you have a chronic condition. The team also meets to talk about your care.
  • The most important member of your Care Team is you because you direct your own care. Say the kind of help you want. The team suggests services that can help. Then you decide which ones to try.

HPSM's Care Management program is voluntary. You can opt in or opt out at any time: it will not affect your benefits.
