CareAdvantage members assigned to PAMF for primary care are invited to join Grove San Mateo. At Grove, a primary care team of PAMF and HPSM clinicians will work with you to understand and respond to your individual needs. This can better help you reach your health goals with:
- Access to all PAMF specialists because you’ll still be a PAMF patient after joining Grove.
- A group of specialists who provide coordinated care.
- On-demand health care appointments, including same-day, urgent and home visits. (You can get home visits if you are homebound or have extreme difficulty getting to the clinic.)
- Calls answered by someone on your care team, not a call center.
- All your care at one clinic located in the city of San Mateo.
