  • Services must be obtained from a CareAdvantage network optometrist or ophthalmologist
  • Every year, choose between one pair of eyeglasses with frames or contact lenses
  • $175 allowance for frames or lenses every year

Use the online provider search form to find the latest information for in-network primary care providers (PCP), specialists, pharmacies, clinics or hospitals. This directory is updated weekly.  You may need a referral from your PCP before making an appointment with a specialist for non-emergency care. Call your PCP to find out if a referral is required.

New address or contact information?

If you move and/or change your contact information (phone number, email), you must tell both HPSM and either the San Mateo County Human Services Agency at 1-800-223-8383 or Social Security Administration at 1-800-772-1213. This will ensure that you continue to get important information about your Medi-Cal coverage status.

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
