Community-Based Adult Services

Community-Based Adult Services (CBAS) is a health-oriented daycare program that helps older adults and people with disabilities live independently and avoid unnecessary institutionalization. At HPSM’s 10 CBAS centers in San Mateo, San Francisco and Santa Clara counties, members can access a range of therapeutic services and social activities in one convenient location, including:

  • Individual assessments – CBAS staff conduct a comprehensive three-day evaluation of each new participant and recommend support services based on their specific needs
  • Onsite therapeutic services provided by professional clinicians
    • Professional nursing services
    • Physical, occupational and speech therapies
    • Mental health services
    • Social services
    • Nutritional counseling
  • Personal care and meals
  • Daily recreational activities – options vary by CBAS center, but can include games, live entertainment, movies, crafts and gardening
  • Opportunities to socialize with other program participants in a safe, relaxed atmosphere – making CBAS centers a great place to meet people and make new friends

Door-to-door van transportation to and from CBAS centers is available. Participants must be at CBAS a minimum of four hours per day and at least two days per week. Family/caregiver training and support is also offered.

Eligibility for CBAS

CBAS is available to eligible Medi-Cal and CareAdvantage beneficiaries with chronic medical, cognitive or mental health conditions and/or disabilities that are at risk of needing institutional care. HPSM members must meet specified medical necessity criteria and at least one of the following:

  • Nursing Facility Level of Care A (NF-A) guidelines or above
  • Developmental disability
  • Moderate to severe Alzheimer’s disease or other type of dementia 
    and
  • Mild cognitive impairment such as moderate Alzheimer's disease or other type of dementia that necessitates assistance or supervision with two self-care activities*
  • Chronic mental illness or brain injury and need assistance or supervision with two self-care activities* and one of the following: money management, accessing resources, meal preparation or transportation

*Self-care activities include bathing, dressing, self-feeding, toileting, ambulation, transferring, medication management and hygiene

Applying to CBAS

If you are an HPSM member or member's representative who could benefit from the program please call the CBAS center that you are interested in. They will determine your or the member’s preliminary eligibility for the program. Then you can schedule a tour of the facility, after which you can apply.
