Beginning May 1, 2026, the Health Plan of San Mateo (HPSM) will pay an additional $3 per member per month to support primary care in our network. The “Population Health Management Payment” is intended to support practices in further developing population health management capabilities. If you are a primary care provider contracted for Medi-Cal and receiving capitation payments from HPSM, you will receive this additional temporary payment.
This payment is one of the latest investments in our primary care network as part of HPSM’s Primary Care Investment Strategy. HPSM provides both capacity-building resources (e.g., Primary Care Provider Grants and Primary Care ACT [Assessments, Coaching and Technical Assistance]), as well as financial resources (e.g., Population Health Management Payment) to support population health management capabilities across our network.
