HPSM launched its first pay for performance (P4P) program for Medi-Cal in January 2008. This quality initiative improves care by aligning preventive care providers want to provide, as recommended by evidence-based best practices, with financial incentives that demonstrate HPSM values the time providers spend on delivering preventive care to our Medi-Cal members. For some P4P measures, providers are required to complete and fax forms to HPSM to receive their incentive payments. Specific CPT codes and modifiers may also be required when billing HPSM. All forms must be faxed to (650) 829-2009.
