Benchmark P4P Incentive

HPSM's Benchmark Pay-for-Performance (P4P) incentive offers bonus payments to in-network primary care providers for meeting performance targets in select quality metrics. The program is designed to improve outcomes by sharing data with HPSM primary care clinics on care gaps and quality performance for their assigned patients.

Providers who enroll in HPSM’s Benchmark P4P Programs for Primary Care program are not eligible to participate in the Care Gap Closure P4P Incentive Program.

Guidelines and measures

Review programmatic information in the Program Guidelines, including clinic eligibility, important dates and deadlines, and quality metrics sets and their technical specifications.

Care Gap Closure P4P Incentive

HPSM is sponsoring a new population health management platform for primary care providers in partnership with Stellar Health. Providers enrolled in our Care Gap Closure Pay-for-Performance (P4P) Incentive program will have access to this web-based platform at no cost to assist in the identification of care gaps. This platform offers real-time bonus payments rewarding providers for individual actions (e.g. scheduling appointments, submitting claims) and efforts to close priority care gaps in well visits, initial health appointments, preventive screenings, medication adherence, immunizations and more.

Providers may begin accessing the Stellar platform on February 1, 2024. Providers must participate in an onboarding call with the Stellar team to gain access. All contracted primary care clinics will receive scheduling outreach from Stellar between December 2023 – June 2024. If you would like to receive priority outreach from Stellar, please complete this form. Early adoption of the Stellar platform by March 31, 2024 will qualify your practice for an additional 20% bonus earnings through the end of the 2024 calendar year.

Providers who enroll in Stellar are not eligible to participate in HPSM’s Benchmark Pay-for-Performance Programs for Primary Care program.
