Your monthly payment (premium) and co-payments for services depend on family income and family size.
- Monthly premium per child is between $4 and $21. The maximum amount for all children in one family is $63.
- Co-pay for some services is $0, and between $5 and $15 for others. The maximum amount a family pays in co-payments
for all children enrolled in Healthy Families is $250 for one benefit period. A benefit period starts October 1 and
ends September 30 of the following year.
