CareAdvantage will only cover services you receive from providers in the plan network.
You are covered for services from a non-network provider in the United States only in these situations: medical
emergencies, urgent care outside of the plan’s service area, or CareAdvantage pre-approves coverage for a visit
to non-network provider. Referrals are required for some types of specialists, and are not required for other types.
