How to Enroll and Disenroll

Call the CareAdvantage Unit to request an enrollment or disenrollment form in the mail. You can also download and print these forms.

   How do I enroll if I am eligible?
   Call the CareAdvantage Sales Department
at 1-888-252-3153 or 650-616-1500.
You can make an appointment to enroll in person in one of two ways:
  • An enrollment specialist can come to your home,
    or
  • You can come to HPSM's office

You can choose one of these ways to enroll:
   Mail a completed enrollment form to the CareAdvantage Unit
   Or enroll online through the official Medicare enrollment website
CMS Online Enrollment Center (OEC).

  How do I disenroll from CareAdvantage (HMO)?

To disenroll you must contact the Health Plan of San Mateo or Medicare about your decision to leave. Choose one of these ways:

   Fax or mail a disenrollment form to the CareAdvantage Unit. Be sure to sign and date your form. If you fax this form, you still need to send the original copy.

   Call the national Medicare helpline, 1-800-MEDICARE (1-800-633-4227),
24 hours a day, 7 days a week. TTY users should call 1-877-486-2048.

CareAdvantage will send you a letter that tells you the date when your membership ends (disenrollment date).

   You cannot receive medical care from providers outside of CareAdvantage until the effective date of disenrollment.

   Call us to verify your disenrollment date before you receive medical services outside of CareAdvantage’s network.

Who else can I call for questions about my rights?

24 hours a day, 7 days a week. TTY users should call 1-877-486-2048.

   San Mateo Health Insurance Counseling and Advocacy Program Office (HICAP) 650-627-9350 or 1-800-434-0222.

They provide free counseling to Medicare beneficiaries, disabled adults, seniors and their families or other representatives on how the health care system works and how to get services.
