Care Coordination
The HPSM Care Coordination Program proactively coordinates complex care to help facilitate the best clinical and functional outcomes for our members. Program staff are knowledgeable of each member’s benefits and work to facilitate optimal use of those benefits.
Complex Care Management PCP Referral
We encourage primary care physicians to refer cases to the Care Coordination department for assistance with coordination of care and linkages to community resources.
Cases for care coordination are identified through:
- health assessments
- provider referral
- data source analysis
Care Coordination staff work with:
- members and their families
- primary care physicians and specialists
- community services
